INSTRUCTIONS
FOR
Certificate
of Doing Business Under Assumed Name ("D/B/A")
Every person who conducts
business in Dutchess County under an assumed name (such as John's Auto Parts)
is required to file a “Certificate of Doing Business Under Assumed Name” form
with the County Clerk's Office. This
form is used for a DBA by individuals.
If the business is to be incorporated (including the words corporation,
incorporated or limited), it must be filed in Albany.
Required:
v Prior
to filing, check to see if business name is already on file in County's Records
Room
v Name
and address of the business (a PO Box is not sufficient)
v Full
name, address and signature of the individual conducting business
v Include
the age of any signer who is under 18 years old
v Completed
form must be signed in the presence of a Notary Public or Commissioner of Deeds
v Bring
photo ID if signature is to be witnessed (at no charge) at County Clerk's
Office
v $25
in cash, money order or certified check made payable to Dutchess County Clerk
for filing; add an additional $5 per certified copy
v Mail
notarized form or bring to:
